Blue Ribbon Rules for Healthy Children in School
1. Make sure immunizations are up to date. MN. law prohibits
attendance without the proper vaccinations. We need the dates of
those summer shots. Students starting Kindergarten need to be
current on their immunizations. When your child receives their
immunizations please mail or drop them off at school so the dates
can be entered in their chart before school starts.
Students entering 7th grade need the required DT, MMR &
Hepatitis B injections. They also need proof that they have had the
chicken-pox and if they have not, the chicken-pox vaccine is
required. If they do not have these shots they will not be
permitted to attend school until they receive their injections.
2. Get medications refilled, permission forms signed and bring them
and the medications in the original pharmacy bottle into health
services on or before the first day of school. This includes
inhalers, and epi-pens too.
3. Tell us of any new health concern or changed condition of your
child.
4. Tell us of new phone numbers or work numbers. We must be able to
reach you or another responsible adult in the case of an
emergency.
5. Check your children’s head for lice before school starts.
6. Remember to keep sick children home. They should have a normal
temperature for 24 hours before returning to school. Sending them
back too early spreads the illness to others and can result in a
relapse for the sick child.
7. Remember we are not a Clinic or Emergency room. Our role is to
care for children who become sick or injured at school. Injures and
illness occurring at home or over the weekend need to be diagnosed
by your health care provider.
8. Sports physicals need to be current and the Minnesota State High
School League and Osakis permission forms must be filled out and
signed by both parents and students before your student can
participate in their first game. These forms will be given to your
child at the first day of practice or can be downloaded from this
website.
9. Please remember to send healthy snacks with your child for their
milk breaks. No candy or junk food please.

• Athletic physical form
Athletic physicals are good for three years. Typically all
students entering grades seven and ten need a physical. Students
new to the district need to have record of their physical
transferred to Osakis Public Schools or have another physical.
